First Visit

Your initial appointment will consist of a comprehensive exam and consultation explaining your diagnosis and treatment options. Occasionally, treatment can be performed the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on a separate day.

Please assist us by providing the following information at the time of your consultation:

• X-rays: If you have X-rays from a previous dentist, you may request that they forward them to our office. Please note, that we are paperless (environmentally friendly) office. Your X-rays can be e-mailed to us. Since all our imaging is digital there is no problem with taking new AND CURRENT X-rays at our office.

• List of any medical conditions you may have from you Physician and a Medical Clearance Letter.
• A list of your current medications
• If you have medical or dental insurance, bring your card/information and the necessary completed forms. This will save time and allow us to help you process any claims.
